Typically this error come from duplicate key existence.
Please check if there are more than one users having same value in mail attribute.

Hello,
our Office 365 Agent got this error for some specific user.
[2013-06-19 11-35-16]Trying to find target with Filter [(userPrincipalName=PWSyncServer@domain.com)]
[2013-06-19 11-35-16]terryh@domain.com : mail : domain.com : PWSyncServer@domain.com : Item has already been added. Key in dictionary: 'terryh@domain.com' Key being added: 'terryh@domain.com'
It would be appreciated if anyone help me.
